JERICHO PROJECT
JOB DESCRIPTION


Position: Case Manager
Reports To: Assistant Program Director
Location: Jericho House

2013 Adam Clayton Powell, Jr. Blvd.

New York, NY 10027

Hours: Full-Time 3:00pm-11:00pm Sunday—Thursday
Responsibilities:


  • Carry a caseload of 15-20 tenants. The Case Manager will meet with each tenant on his/her caseload a minimum of one time per month to:

  • Develop a Service Plan

  • Provide individual counseling

  • Provide crisis management and interventions

  • Assist the Program Director and Assistant Program Director with intake process

  • Attend house meetings, case conferences, and staff meetings

  • Comply with charting requirements as mandated by our funding sources

  • Maintain linkages with community-based organizations such as shelters and halfway houses for possible referrals

  • Assume responsibility for the overall running of the facility during the evening hours when the Assistant Program Director

  • Serve as Jericho House’s representative on the agency’s Events Committee

  • Periodically attend Community Board and other local meetings

  • Help recruit and manage volunteer group leaders




  • Qualifications:




  • A Bachelor’s Degree in Social Work or related field is preferred. Will also consider a candidate with comparable training and extensive work experience in the field. Must have working knowledge of homelessness and substance abuse. Must possess superior written and verbal skills. Must be computer literate.
