I understand that Federal Law Enforcement Authorities will perform a background check. I understand there are severe penalties for attempting to fraudulently enter the Air Force. My application accurately reflects all of the following items. Initial next to each item:

___ Law Involvement: I have revealed ALL Juvenile, Criminal and Traffic violations, to include Teen Court, whether or not charges were formally filed, were dropped, dismissed, charge was reduced, or I was found not guilty. This includes any charges where the records are sealed, expunged, or stricken or you were told “you don’t have to say anything.” Have you ever talked to a police officer, judge, probation officer, etc? Have you ever been in a court house or police station? Have you ever been involved with law enforcement and someone told you “it would be taken care of?” Are ALL listed in your application? Yes or No

___ Pending Actions: I do not have any open court actions of any kind. This includes unpaid fines, supervised or unsupervised probation, court ordered actions such as community service, or any open civil cases.

___ Drug Abuse: I have revealed all involvement with illegal drugs, including marijuana, and any abuse of prescription or over the counter medications. I have not used marijuana in the last 45 days? Yes or No

___ Dependents: My application accurately reflects my marital and dependency status. I have told my recruiter about any possible changes such as anticipated marriage or expected birth of a child. Males: Is anyone pregnant by you? Females: Are you pregnant? Yes or No

___ Medical/ Psychological: My application includes my entire medical/psychological history, regardless of how trivial or sensitive the information, for every case where I was seen, evaluated, or treated by a medical or psychological (counselor, social worker, psychologist, psychiatrist or mental health professional) authority.

I understand the applicant’s casefile must be completed and released to the MEPS at least 24 hours prior to processing. I also understand the applicant’s Birth Certificate, Citizenship Documents- I-551, etc. (if applicable), SS Card, Education Verification Documents, and Parental Consent (if applicable) must be e-mailed/faxed to the MEPS at least 24 hours in advance. A 680-AE will be included if the applicant tested or processed for another service. My applicant understands the entire process. I will complete this checklist with every applicant prior to each processing trip to the MEPS. I understand that this is a MANDATORY briefing and ALL applicants will hand carry this form to the MEPS on processing day and they must also have the original source documents listed above to process.

No Show

Medical History

DON’T BE INTIMIDATED. Either it happened or didn’t happen….DON”T GUESS about your Medical History. Don’t be a hypochondriac.




Test DQ

Take the test seriously and get plenty of rest




Over Weight

You must be 5 LBS below your maximum allowable weight.




REF RES

If you are qualified and refuse to join, your application will be closed and will affect future application acceptance.




Open Law or Civil Cases

Ensure you have told your RECRUITER about every law violation. If you are required to go to Court or a Hearing for any reason. LET YOUR RECRUITER KNOW! (Example: If you are required to go to a Suspension hearing for points on your License) OR If you are required to go to court for any civil matter (bankruptcy, divorce, etc.) let your recruiter know.




Law DQ

Ensure you have told your recruiter about every law violation. Have you ever talked to a police officer, judge, probation officer, etc? Have you ever been in a court house or police station? Asthma

Do not be intimidated into saying you have asthma if you Don’t




Counseling

Don’t say anything that can be misconstrued. If in doubt, ask questions before you go to the MEPS.




Drugs

DO NOT GO UP to MEPS if you have smoked marijuana in the last 45 Days




Tattoos/Body Piercing

If you have any tattoos or piercings, let your recruiter KNOW NOW!

WARNING: Once you are qualified, any new tattoos or piercing may disqualify you




Inhaler Use

Don’t say anything that can be misconstrued




No PC

If you are 17, ensure you have a valid Parental consent and your 2807 (all dates correct) is signed




Dependency

Ensure you have told your recruiter about marriages, past marriages, ANY KIDS!




Eyes

IF you have glasses BRING them, best to have an “eye form” or current prescription




Allergies

Don’t say anything that can be misconstrued…..if you don’t have, don’t say you do




Credit/FED

IF 23 or ever married, Does your recruiter have an approved credit check or FED.? If you have any financial problems (collection accounts, unfulfilled contracts, etc.), no matter how small, let your recruiter know




Education

Do you have the proper documentation to show you have or will graduate




On Medicine

If you are currently on any medication, ensure you have told your Recruiter…most are disqualifying




Recent Surgery

If you had any recent surgery, ensure you have told your Recruiter




Migraines

Tell your recruiter if you ever had migraines…have you seen a doctor for headaches? & Don’t say anything that can be misconstrued




Amnesia

How would you actually remember if you ever had amnesia?




Cold

If you have a serious cold, you may not want to go to MEPS yet




Cysts

Tell your recruiter if you ever had cysts & Don’t say anything that can be misconstrued




Back Pain

Tell your recruiter if you ever had back pain…Have you seen a doctor for back problems? & Don’t say anything that can be misconstrued




Mental Health

Tell your recruiter if you ever talked to a counselor, social worker, psychologist, psychiatrist or mental health professional (NOTE: Psychological information will be handled in a very private and professional manner.)& Don’t say anything that can be misconstrued




Blisters on Feet

Tell your recruiter if you have any open blisters or sores & Don’t say anything that can be misconstrued




Recent Abortion/Miscarriage

Tell your recruiter if you had an abortion/miscarriage in the last 12 months




Bronchitis

Tell your recruiter if you ever had Bronchitis & Don’t say anything that can be misconstrued




Under Weight

If close, remember liquid intake, a glass of water weighs almost a pound




Braces letter

If you have braces on or any kind of appliances, ensure you have a letter




No ID

Remember to Bring your OFFICIAL ID with you, show your recruiter it




Citizenship

Must have INS 551 or appropriate document




HS Lookup required

IF you took the ASVAB in HS, ensure your recruiter did a HS Lookup




Missing Docs

Bring ALL your documents: Diploma, Birth Cert, SSAN card




Improper Shoes, Clothing, and Underclothing

Wear proper pants, shirt, shoes, socks, and underwear




Weapons

No weapons (knives, guns, brass knuckles, etc.) will be taken to the MEPS
